What goes in each box?
Use a woman's maiden name, the name she was born with, because it's the name her parents will be found under. Write a date like 12 Mar 1891, so nobody mixes up the day and the month, and write "about" when you're guessing. Rather use paper? Print a blank template.
Stuck on a name?
Ask the oldest relative you have, then see how to find your great-grandparents and the questions to ask.
Your family may already be drawn.
FamilySearch Family Tree is one tree that everyone shares, and it's free, so someone in your family may already have started yours. Ancestral Fire brings it in, draws it far past four generations, and tells you who those people were, with the source behind each fact. You can bring in the file you download here too.
